Before you come
into our office, you need to know...
- Our goal is to provide you with the finest orthopedic
care and service. Before beginning treatment we will discuss
your case with you in detail and try to answer all of your
questions. Please bring with you any test results including
x-rays, bone scans, or MRIs pertaining to your injury.
- For shoulder exams: Females please wear or bring a sleeveless
shirt or tank top for the exam.
- For hip, knee, leg, ankle, or foot exams: Please wear or bring a pair of shorts.
Financial Policies:
- To prevent any misunderstanding, we would like to take
this opportunity to inform you of our financial policies.
As a courtesy, we will file your insurance claim after your
visit. In order to provide this service, we must have complete
insurance information for primary and secondary coverage.
Most insurance carriers require that you have a written
referral from your primary care physician before seeing
us. You should bring this referral form with you. It is
also your responsibility to pay your co-payment.
- If you are not covered by an insurance plan, you will
be responsible for payment on your date of service. We accept
Visa, Mastercard, and American Express for you convenience.
- If there is any way we can assist you regarding our financial
policies, please contact our Patient Accounts Representative
at 570-7272 PRIOR to your visit.
Insurance:
- Most private Insurance Plans (please verify with your
insurance company)
- Most Workers Compensation
- Medicare
Our Office Hours:
- Monday to Friday: 8:00am to 5:00pm
- Closed Saturday, Sunday and Holidays
Pre-Surgical Preparation:
- Nothing to eat or drink 8 hours prior to surgery except
for approved meds with a sip of water
- Arrange a ride home if you are scheduled for outpatient surgery.
- Notify your health plan and family physician of your upcoming surgery.
- Fill pain meds preoperatively if appropriate
Post Surgical Information:
- Follow discharge instruction
- Ice and elevate the operated extremity
- Plan to rest for a few days after surgery to minimize
bleeding and swelling
- Remember to call to renew your narcotic prescriptions
during regular office hours. This minimizes post-operative
confusion and delay in refilling your prescription
- IMPORTANT: If a postoperative
emergency develops, call 9-1-1, or go to the nearest emergency
room. Non-emergency postoperative questions will be answered
by the on-call physician. Please call our office at 570-7272
